The Chapel St Group are developing programmes that support community health and wellbeing. Our health initiatives include projects in both Blackburn and London.

Community Health Online in Blackburn

Chapel St created a new community health web site for the Mill Hill area of Blackburn. The web site connects people with local groups, clubs and services that enhance community life. Chapel St worked with Blackburn with Darwen Primary Care Trust (PCT) and Cornerstone Healthcare to support to a GP practice (the Bentham Road Medical Centre) in the delivery of this Community Oriented Primary Care (COPC) pilot. Cassidy Medical Centre and Health Hub

The Cassidy Medical Centre and Health Hub provides full GP and extra health services to the community of Fulham. It is an NHS practice, delivered by Chapel St Health Company with The Hurley Group. Chapel St Community Fund works to extend the reach of the GP practice more deeply in to community life and to enhance wellbeing in the community. MumPlusMum project is reaching isolated and vulnerable mums in the area.
