Chapel St are developing programmes that support community health and wellbeing. Our health initiatives include:

Cassidy Medical Centre and Health Hub

Cassidy-Road-Flier-13The Cassidy Medical Centre and Health Hub provides full GP and extra health services to the community. It is an NHS practice, delivered by Chapel St with The Hurley Group.

Chapel St are working to extend the reach of the GP practice more deeply in to community life. We are creating a range of programmes to enhance wellbeing in the community.

MumPlusMum

MumPlusMum social media iconChapel St are developing MumPlusMum – a new mentoring scheme for women during their pregnancy and early motherhood. We know that having a baby brings big changes to life and offer mentoring for mums-to-be and new mums. Running in Fulham and Roehampton, the project is currently looking for volunteer mums to be mentors. All volunteers receive 10 hours of free mentoring training from a quality practitioner. Read more

Link Line

Link Line Logo for webChapel St are developing Link Line – a telephone support and errand service – to run alongside the Cassidy Medical Centre. The service provides care on the phone and to the home. The purpose of the project is to assist people with every day living and enable people to continue to live independently. Read more

Community Health Online in Blackburn

Chapel St created a new community health web site for the Mill Hill area of Blackburn. The web site connects people with local groups, clubs and services that enhance community life. Chapel St worked with Blackburn with Darwen Primary Care Trust (PCT) and Cornerstone Healthcare to support to a GP practice (the Bentham Road Medical Centre) in the delivery of this Community Oriented Primary Care (COPC) pilot.
