Chapel St Health Working for Community Health and Well-being
People in deprived areas face too many closed doors when it comes to accessing quality employment, education and financial opportunities, all of which can have an adverse affect on community health. Postcodes and health prospects are linked. At Chapel St, we are working to develop creative ways of enhancing community health and tackling issues that affect health prospects in under-resourced areas. All our health projects focus on combining healthcare excellence with creative community engagement services.
Cassidy Road Health Hub Working for Community Health and Well-being in Fulham
Delivered in partnership with The Hurley Group, our Health Hub on Cassidy Road in Fulham is engaging with an area of the community marked by inadequate housing, high rates of unemployment, alcohol dependence and substance misuse. As well as delivering a high quality GP service, we are working with local people, groups and churches to provide an extra layer of community services – all designed to enhance community health. From support for lone parents, extra family services, improving care for drug and alcohol users or addressing the needs of minority groups, our Health Hub is working to extend the reach of usual GP health provision more deeply into the life of the community.
Community Oriented Primary Care Working for Community Health and Well-being in Blackburn
Chapel St is working for Healthy Happy Communities in the Mill Hill area of Blackburn. Chapel St will be coordinating with Mill Hill residents, community groups, Blackburn with Darwen Primary Care Trust (PCT) and Cornerstone Healthcare to support to a GP practice (the Bentham Road Medical Centre) in the delivery of a Community Oriented Primary Care (COPC) pilot. The aim of the initiative is to engage local communities in the process and delivery of improving health outcomes for the ward. This year-long project will help to redesign local healthcare provision and provide local people with opportunities to enjoy the very best of health as individuals, families and community.
